Yeled v'Yalda's Social Care Network (SCN) program helps eligible New York Managed Medicaid members receive free weekly food deliveries—hot prepared meals, fresh produce, and pantry staples—delivered right to their door, at no cost.
Who Qualifies
You may qualify for weekly food deliveries at no cost if you are enrolled in a New York Managed Medicaid plan and have a health or household need.
Managed Medicaid Plan Requirements
This program is funded through New York Managed Medicaid health plans. Members with the following plans typically qualify:
- Fidelis Care
- Healthfirst
- MetroPlus Health
- EmblemHealth
- UnitedHealthcare Community Plan
- Other Managed Care Plans
Original / Straight Medicaid: Members on Original Medicaid can receive this benefit by switching to a Managed Care plan. Switching is free and keeps your same doctor preferences.
Qualifying Health & Life Situations
You or a family member in your household may qualify if any of the following apply:
- Pregnancy & Postpartum — Pregnant individuals, or mothers who gave birth within the past 12 months.
- Chronic Health Conditions — Ongoing diagnoses such as Diabetes, High Blood Pressure, Heart Disease, or Asthma.
- Child Health & Developmental Needs — Families with children receiving health, learning, or developmental support services.
- Housing, Transit & Social Needs — Individuals needing help with housing support, medical transport, or other community resources.

How It Works
- Apply. Complete one short online form. Takes about five minutes. No paperwork to mail.
- We Review. Our onboarding team reads your application within one business day and flags it for SCN.
- SCN Verifies. Our partner confirms your Medicaid eligibility with NY State. Typically 3–5 business days.
- Deliveries Begin. We call you in English or Spanish to confirm your details, and weekly food deliveries start arriving.
